Motrin Sinus Headache


Generic Name: ibuprofen and pseudoephedrine (EYE bue pro fen and SOO doe ee FED rin)

What is Motrin Sinus Headache (ibuprofen and pseudoephedrine)?

Ibuprofen is nonsteroidal anti-inflammatory drugs (NSAID) that reduces hormones that cause inflammation and pain in the body.


Pseudoephedrine is a decongestant that shrinks blood vessels in the nasal passages. Dilated blood vessels can cause nasal congestion (stuffy nose).


The combination of ibuprofen and pseudoephedrine is used to treat stuffy nose, sinus congestion, cough, and pain or fever caused by the common cold or flu.


Ibuprofen and pseudoephedrine may also be used for purposes not listed in this medication guide.


What is the most important information I should know about Motrin Sinus Headache (ibuprofen and pseudoephedrine)?


Always ask a doctor before giving a cough or cold medicine to a child. Death can occur from the misuse of cough and cold medicines in very young children. Do not use ibuprofen and pseudoephedrine if you have used an MAO inhibitor such as furazolidone (Furoxone), isocarboxazid (Marplan), phenelzine (Nardil), rasagiline (Azilect), selegiline (Eldepryl, Emsam, Zelapar), or tranylcypromine (Parnate) within the past 14 days.

Ibuprofen may cause life-threatening heart or circulation problems such as heart attack or stroke, especially if you use it long term. Do not use this medication just before or after heart bypass surgery (coronary artery bypass graft, or CABG).


Get emergency medical help if you have chest pain, weakness, shortness of breath, slurred speech, or problems with vision or balance.

Ibuprofen may also cause serious effects on the stomach or intestines, including bleeding or perforation (forming of a hole). These conditions can be fatal and can occur without warning while you are taking ibuprofen, especially in older adults.


Call your doctor at once if you have symptoms of stomach bleeding such as black, bloody, or tarry stools, or coughing up blood or vomit that looks like coffee grounds. Do not take more of this medication than is recommended. An overdose of ibuprofen can cause damage to your stomach or intestines.

What should I discuss with my doctor before taking Motrin Sinus Headache (ibuprofen and pseudoephedrine)?


Do not use this medication just before or after heart bypass surgery (coronary artery bypass graft, or CABG).


Ibuprofen may cause life-threatening heart or circulation problems such as heart attack or stroke, especially if you use it long term.


Ibuprofen may also cause serious effects on the stomach or intestines, including bleeding or perforation (forming of a hole). These conditions can be fatal and can occur without warning while you are taking ibuprofen, especially in older adults.

A dangerous drug interaction could occur, leading to serious side effects. You should not use this medication if you are allergic to ibuprofen or pseudoephedrine, or if you have:

  • a stomach ulcer or active bleeding in your stomach or intestines;




  • polyps in your nose; or




  • a history of allergic reaction to aspirin or other NSAIDs.



Ask a doctor or pharmacist if it is safe for you to take this medicine if you have:



  • a history of stomach ulcers or bleeding;




  • heart disease, congestive heart failure, high blood pressure;




  • systemic lupus erythematosus (SLE);



  • liver or kidney disease;


  • a thyroid disorder;




  • diabetes;




  • enlarged prostate or problems with urination;




  • a bleeding or blood clotting disorder; or




  • if you smoke.




Taking ibuprofen during the last 3 months of pregnancy may result in birth defects and prolonged labor and delivery. Do not take this medication without medical advice if you are pregnant. It is not known whether ibuprofen and pseudoephedrine passes into breast milk or if it could harm a nursing baby. Do not use this medication without telling your doctor if you are breast-feeding a baby.

How should I take Motrin Sinus Headache (ibuprofen and pseudoephedrine)?


Use exactly as directed on the label, or as prescribed by your doctor. Do not use in larger or smaller amounts or for longer than recommended. Cold medicine is usually taken only for a short time until your symptoms clear up.


An overdose of ibuprofen can damage your stomach or intestines. Adults should not take more than 800 milligrams per dose or 3200 mg per day (4 maximum doses).


Do not give this medication to a child younger than 4 years old. Always ask a doctor before giving a cough or cold medicine to a child. Death can occur from the misuse of cough and cold medicines in very young children. Shake the oral suspension (liquid) well just before you measure a dose. Measure the liquid with a special dose-measuring spoon or medicine cup, not with a regular table spoon. If you do not have a dose-measuring device, ask your pharmacist for one. Call your doctor if you have a fever lasting longer than 3 days, if you have new symptoms, or if your condition does not improve after taking this medication for 7 days.

If you need surgery, tell the surgeon ahead of time that you are using ibuprofen and pseudoephedrine. You may need to stop using the medicine for a short time.


Store at room temperature away from moisture and heat.

What happens if I miss a dose?


Since cold medicine is usually taken only as needed, you may not be on a dosing schedule. If you are taking the medication regularly, take the missed dose as soon as you remember. Skip the missed dose if it is almost time for your next scheduled dose. Do not take extra medicine to make up the missed dose.


What happens if I overdose?


Seek emergency medical attention or call the Poison Help line at 1-800-222-1222.

Overdose symptoms may include nausea, vomiting, stomach pain, dizziness, feeling restless or nervous, blurred vision, sweating, breathing problems, or seizure (convulsions).


What should I avoid while taking Motrin Sinus Headache (ibuprofen and pseudoephedrine)?


Ask a doctor or pharmacist before using any other cough, cold, or pain medicine. Ibuprofen and pseudoephedrine are contained in many combination medicines. Taking certain products together can cause you to get too much of a certain drug. Check the label to see if a medicine contains ibuprofen or pseudoephedrine. Avoid drinking alcohol. It may increase your risk of stomach bleeding.

Motrin Sinus Headache (ibuprofen and pseudoephedrine) side effects


Get emergency medical help if you have any of these signs of an allergic reaction: hives; difficulty breathing; swelling of your face, lips, tongue, or throat. Stop taking this medication and call your doctor at once if you have a serious side effect such as:

  • chest pain, weakness, shortness of breath, slurred speech, problems with vision or balance;




  • bloody, or tarry stools, coughing up blood or vomit that looks like coffee grounds;




  • fast, pounding, or uneven heartbeat;




  • severe dizziness, anxiety, restless feeling, or nervousness;




  • easy bruising or bleeding, unusual weakness, fever, chills, body aches, flu symptoms;




  • dangerously high blood pressure (severe headache, buzzing in your ears, confusion, chest pain, shortness of breath);




  • urinating less than usual or not at all;




  • skin rash, severe tingling, numbness, pain, muscle weakness; or




  • fever, headache, neck stiffness, chills, increased sensitivity to light, purple spots on the skin, and/or seizure (convulsions).



Less serious side effects may include:



  • upset stomach, nausea, heartburn, diarrhea, constipation;




  • bloating, gas, loss of appetite;




  • warmth, tingling, or redness under your skin;




  • dizziness, headache, feeling excited or restless;




  • sleep problems (insomnia); or




  • mild itching or skin rash.

What other drugs will affect Motrin Sinus Headache (ibuprofen and pseudoephedrine)?


Tell your doctor about all other medicines you use, especially:



  • a blood thinner such as warfarin (Coumadin, Jantoven);




  • steroids (prednisone and others);




  • diuretics (water pills), or medicines to treat high blood pressure;




  • a beta-blocker such as atenolol (Tenormin, Tenoretic), carvedilol (Coreg), labetalol (Normodyne, Trandate), metoprolol (Dutoprol, Lopressor, Toprol), nadolol (Corgard), propranolol (Inderal, InnoPran), sotalol (Betapace), and others;




  • an antidepressant such as amitriptyline (Elavil, Vanatrip, Limbitrol), doxepin (Sinequan), nortriptyline (Pamelor), and others; or




  • aspirin or other NSAIDs (non-steroidal anti-inflammatory drugs) such as ibuprofen (Advil, Motrin), naproxen (Aleve, Naprosyn, Naprelan, Treximet), celecoxib (Celebrex), diclofenac (Arthrotec, Cambia, Cataflam, Voltaren, Flector Patch, Pennsaid, Solareze), indomethacin (Indocin), meloxicam (Mobic), and others.

Uses For Aricept


Donepezil is used to treat dementia (memory loss and mental changes) associated with mild, moderate, or severe Alzheimer's disease. Donepezil will not cure Alzheimer's disease, and it will not stop the disease from getting worse. However, it can improve thinking ability in some patients.


This medicine is available only with your doctor's prescription.


Before Using Aricept


In deciding to use a medicine, the risks of taking the medicine must be weighed against the good it will do. This is a decision you and your doctor will make. For this medicine, the following should be considered:


Allergies


Tell your doctor if you have ever had any unusual or allergic reaction to this medicine or any other medicines. Also tell your health care professional if you have any other types of allergies, such as to foods, dyes, preservatives, or animals. For non-prescription products, read the label or package ingredients carefully.


Pediatric


Appropriate studies have not been performed on the relationship of age to the effects of donepezil in the pediatric population. Safety and efficacy have not been established.


Geriatric


Appropriate studies performed to date have not demonstrated geriatric-specific problems that would limit the usefulness of donepezil in the elderly.


Pregnancy








Pregnancy CategoryExplanation
All TrimestersCAnimal studies have shown an adverse effect and there are no adequate studies in pregnant women OR no animal studies have been conducted and there are no adequate studies in pregnant women.

Breast Feeding


There are no adequate studies in women for determining infant risk when using this medication during breastfeeding. Weigh the potential benefits against the potential risks before taking this medication while breastfeeding.


Interactions with Medicines


Although certain medicines should not be used together at all, in other cases two different medicines may be used together even if an interaction might occur. In these cases, your doctor may want to change the dose, or other precautions may be necessary. When you are taking this medicine, it is especially important that your healthcare professional know if you are taking any of the medicines listed below. The following interactions have been selected on the basis of their potential significance and are not necessarily all-inclusive.


Using this medicine with any of the following medicines is usually not recommended, but may be required in some cases. If both medicines are prescribed together, your doctor may change the dose or how often you use one or both of the medicines.


  • Succinylcholine

Using this medicine with any of the following medicines may cause an increased risk of certain side effects, but using both drugs may be the best treatment for you. If both medicines are prescribed together, your doctor may change the dose or how often you use one or both of the medicines.


  • Oxybutynin

  • Ramelteon

  • Tolterodine

Interactions with Food/Tobacco/Alcohol


Certain medicines should not be used at or around the time of eating food or eating certain types of food since interactions may occur. Using alcohol or tobacco with certain medicines may also cause interactions to occur. Discuss with your healthcare professional the use of your medicine with food, alcohol, or tobacco.


Other Medical Problems


The presence of other medical problems may affect the use of this medicine. Make sure you tell your doctor if you have any other medical problems, especially:


  • Asthma, history of or

  • Lung disease, history of or

  • Peptic ulcers, or history of or

  • Seizures, or history of or

  • Urinary tract blockage or difficult urination—Donepezil may make these conditions worse.

  • Heart problems—Donepezil may have unwanted effects on heart rate.

  • Liver disease or

  • Low body weight (below 50 kilograms or 110 pounds)—Higher blood levels of donepezil may result and increase the chance of side effects.

Proper Use of Aricept


Take this medicine exactly as directed by your doctor in order to improve your condition as much as possible. Do not take more of it or less of it, and do not take it more or less often than your doctor ordered.


Donepezil should be taken at bedtime unless otherwise directed by your doctor. It may be taken with or without food, on a full or empty stomach.


Swallow the tablet whole. Do not crush, break, or chew it.


For patients using the oral disintegrating tablet form of this medicine:


  • Make sure your hands are dry.

  • Do not push the tablet through the foil backing of the package. Instead, gently peel back the foil backing and remove the tablet.

  • Immediately place the tablet on top of the tongue.

  • Do not swallow the tablet whole. The tablet will dissolve in seconds, and you may swallow it with your saliva. You should drink a glass of water after the tablet has dissolved.

For patients taking the oral solution form of this medicine:


  • Shake the bottle well before measuring the dose.

  • Use a specially marked measuring spoon, a plastic syringe, or a small marked measuring cup to measure each dose accurately. The average household teaspoon may not hold the right amount of liquid.

  • If your dose is 5 mg, you should take 5 mL of this medicine.

  • If your dose is 10 mg, you should take 10 mL of this medicine.

Dosing


The dose of this medicine will be different for different patients. Follow your doctor's orders or the directions on the label. The following information includes only the average doses of this medicine. If your dose is different, do not change it unless your doctor tells you to do so.


The amount of medicine that you take depends on the strength of the medicine. Also, the number of doses you take each day, the time allowed between doses, and the length of time you take the medicine depend on the medical problem for which you are using the medicine.


  • For oral dosage forms (oral disintegrating tablets, oral solution, and tablets):
    • For mild to moderate Alzheimer's disease:
      • Adults—5 milligrams (mg) taken at bedtime. Your doctor may increase your dose as needed. However, the dose usually is not more than 10 mg per day.

      • Children—Use and dose must be determined by your doctor.


    • For moderate to severe Alzheimer's disease:
      • Adults—10 milligrams (mg) taken at bedtime. Your doctor may increase your dose as needed. However, the dose usually is not more than 23 mg per day.

      • Children—Use and dose must be determined by your doctor.



Missed Dose


If you miss a dose of this medicine, skip the missed dose and go back to your regular dosing schedule. Do not double doses.


Storage


Store the medicine in a closed container at room temperature, away from heat, moisture, and direct light. Keep from freezing.


Ask your healthcare professional how you should dispose of any medicine you do not use.


Keep out of the reach of children.


Do not keep outdated medicine or medicine no longer needed.


Precautions While Using Aricept


It is important that your doctor check your progress at regular visits. This is necessary to allow dose adjustments and to reduce any unwanted effects.


Before you have any kind of surgery, dental treatment, or emergency treatment, tell the medical doctor or dentist in charge that you are using this medicine. Taking donepezil together with certain medicines that are used during surgery or dental or emergency treatments may increase the effects of those medicines and cause unwanted effects.


This medicine may cause some people to become dizzy or drowsy, to have blurred vision, or to have problems with clumsiness or unsteadiness. Make sure you know how you react to this medicine before you drive, use machines, or do anything else that could be dangerous if you are not alert, well-coordinated, and able to see clearly.


Some people who have used this medicine developed problems such as nausea, severe vomiting, loss of appetite, diarrhea, and weight loss. Talk with your doctor before using this medicine if you have any concerns.


Tell you doctor right away if you have bloody or black tarry stools, constipation, severe stomach pain, or vomiting of blood or material that looks like coffee grounds. These may be symptoms of stomach or bowel bleeding.


If you think that you or someone else may have taken an overdose of this medicine, get emergency help at once. Taking an overdose of this medicine may cause convulsions (seizures) or serious effects on your heart and your breathing. Signs of overdose include increased watering of mouth, increased sweating, low blood pressure, muscle weakness, severe nausea, severe vomiting, slow heartbeat, and troubled breathing.


Do not take other medicines unless they have been discussed with your doctor. This includes prescription or nonprescription (over-the-counter [OTC]) medicines and herbal or vitamin supplements.


Aricept Side Effects


Along with its needed effects, a medicine may cause some unwanted effects. Although not all of these side effects may occur, if they do occur they may need medical attention.


Check with your doctor as soon as possible if any of the following side effects occur:


More common
  • Diarrhea

  • loss of appetite

  • muscle cramps

  • nausea

  • trouble in sleeping

  • unusual tiredness or weakness

  • vomiting

Less common
  • Abnormal dreams

  • constipation

  • dizziness

  • drowsiness

  • fainting

  • frequent urination

  • headache

  • joint pain, stiffness, or swelling

  • mental depression

  • pain

  • unusual bleeding or bruising

  • weight loss

Rare
  • Black, tarry stools

  • bloating

  • bloody or cloudy urine

  • blurred vision

  • burning, prickling, or tingling sensations

  • cataract

  • chills

  • clumsiness or unsteadiness

  • confusion

  • cough

  • decreased urination

  • difficult or painful urination

  • dryness of mouth

  • eye irritation

  • fever

  • flushing of skin

  • frequent urge to urinate

  • high or low blood pressure

  • hives

  • hot flashes

  • increase in sexual desire or performance

  • increased heart rate and breathing

  • increased sweating

  • increased urge to urinate during the night

  • irregular heartbeat

  • itching

  • loss of bladder control

  • loss of bowel control

  • mood or mental changes, including abnormal crying, aggression, agitation, delusions, irritability, nervousness, or restlessness

  • nasal congestion

  • pain in chest, upper stomach, or throat

  • problems with speech

  • runny nose

  • severe thirst

  • shortness of breath

  • sneezing

  • sore throat

  • sunken eyes

  • tightness in chest

  • tremor

  • troubled breathing

  • wheezing

  • wrinkled skin

Incidence not known
  • Back, leg, or stomach pains

  • bleeding gums

  • chest pain or discomfort

  • coma

  • convulsions

  • dark urine

  • difficulty breathing

  • fast or irregular heartbeat

  • fatigue

  • general body swelling

  • general tiredness and weakness

  • high fever

  • increased thirst

  • indigestion

  • light-colored stools

  • muscle pain or cramps

  • nausea and vomiting

  • nosebleeds

  • pains in stomach, side, or abdomen, possibly radiating to the back

  • pale skin

  • rash

  • seeing, hearing, or feeling things that are not there

  • seizures

  • severe muscle stiffness

  • severe nausea

  • slow or irregular heartbeat

  • stomach pain

  • sweating

  • swelling of face, ankles, or hands

  • tiredness

  • unusually pale skin

  • upper right abdominal or stomach pain

  • yellow eyes and skin

Symptoms of overdose
  • Convulsions (seizures)

  • increased sweating

  • increased watering of mouth

  • increasing muscle weakness

  • low blood pressure

  • severe nausea

  • severe vomiting

  • slow heartbeat

  • troubled breathing

What is Medigesic (acetaminophen, butalbital, and caffeine)?

Acetaminophen is a pain reliever and fever reducer.


Butalbital is in a group of drugs called barbiturates. It relaxes muscle contractions involved in a tension headache.


Caffeine is a central nervous system stimulant. It relaxes muscle contractions in blood vessels to improve blood flow.


The combination of acetaminophen, butalbital, and caffeine is used to treat tension headaches that are caused by muscle contractions.


Acetaminophen, butalbital, and caffeine may also be used for purposes not listed in this medication guide.


What is the most important information I should know about Medigesic (acetaminophen, butalbital, and caffeine)?


Do not use acetaminophen, butalbital, and caffeine if you have used an MAO inhibitor such as furazolidone (Furoxone), isocarboxazid (Marplan), phenelzine (Nardil), rasagiline (Azilect), selegiline (Eldepryl, Emsam, Zelapar), or tranylcypromine (Parnate) in the last 14 days. Tell your doctor if you have ever had alcoholic liver disease (cirrhosis) or if you drink more than 3 alcoholic beverages per day. You may not be able to take medicine that contains acetaminophen. Ask a doctor or pharmacist before using any other cold, allergy, pain, or sleep medication. Acetaminophen (sometimes abbreviated as APAP) is contained in many combination medicines. Taking certain products together can cause you to get too much acetaminophen which can lead to a fatal overdose. Check the label to see if a medicine contains acetaminophen or APAP. Avoid drinking alcohol. It may increase your risk of liver damage while taking acetaminophen.

To make sure you can safely take acetaminophen, butalbital, and caffeine, tell your doctor if you have any of these other conditions:



  • kidney disease,




  • liver disease; or




  • a history of mental illness or suicidal thoughts.




Butalbital may be habit forming and should be used only by the person it was prescribed for. Never share this medication with another person, especially someone with a history of drug abuse or addiction. Keep the medication in a place where others cannot get to it. FDA pregnancy category C. It is not known whether acetaminophen, butalbital, and caffeine will harm an unborn baby. Tell your doctor if you are pregnant or plan to become pregnant while using this medication. Acetaminophen, butalbital, and caffeine can pass into breast milk and may harm a nursing baby. Do not use this medication without telling your doctor if you are breast-feeding a baby.

How should I take Medigesic (acetaminophen, butalbital, and caffeine)?


Take exactly as prescribed. Never take acetaminophen, butalbital, and caffeine in larger amounts, or for longer than recommended by your doctor. An overdose of this medication can damage your liver or cause death.Follow the directions on your prescription label. Tell your doctor if the medicine seems to stop working as well in relieving your pain. Take the medicine with food or milk if it upsets your stomach. Store at room temperature away from moisture and heat.

Keep track of the amount of medicine used from each new bottle. Butalbital is a drug of abuse and you should be aware if anyone is using your medicine improperly or without a prescription.


What happens if I miss a dose?


Since this medication is usually taken as needed, you may not be on a dosing schedule. If you are taking the medication regularly, take the missed dose as soon as you remember. Skip the missed dose if it is almost time for your next scheduled dose. Do not take extra medicine to make up the missed dose.


What happens if I overdose?


Seek emergency medical attention or call the Poison Help line at 1-800-222-1222.

The first signs of an acetaminophen overdose include loss of appetite, nausea, vomiting, stomach pain, sweating, and confusion or weakness. Later symptoms may include pain in your upper stomach, dark urine, and yellowing of your skin or the whites of your eyes.


Overdose symptoms may also include insomnia, restlessness, tremor, dizziness, drowsiness, diarrhea, increased sweating, shallow breathing, confusion, uneven heartbeats, seizure (convulsions), or coma.


What should I avoid while taking Medigesic (acetaminophen, butalbital, and caffeine)?


This medication can cause side effects that may impair your thinking or reactions. Be careful if you drive or do anything that requires you to be awake and alert. Avoid drinking alcohol. It may increase your risk of liver damage while taking acetaminophen. Ask a doctor or pharmacist before using any other cold, allergy, pain, or sleep medication. Acetaminophen (sometimes abbreviated as APAP) is contained in many combination medicines. Taking certain products together can cause you to get too much acetaminophen which can lead to a fatal overdose. Check the label to see if a medicine contains acetaminophen or APAP.

While you are taking this medication, avoid taking diet pills, caffeine pills, or other stimulants (such as ADHD medications) without your doctor's advice.

Get emergency medical help if you have any of these signs of an allergic reaction: hives; difficulty breathing; swelling of your face, lips, tongue, or throat. Stop using this medication and call your doctor at once if you have any of these serious side effects:

  • fast, pounding, or uneven heartbeat;




  • feeling light-headed or short of breath;




  • nausea, upper stomach pain, itching, loss of appetite, dark urine, clay-colored stools, jaundice (yellowing of the skin or eyes); or




  • easy bruising or bleeding, unusual weakness, fever, chills, body aches, flu symptoms.



Less serious side effects may include:



  • drowsiness;




  • dizziness, confusion or lightheadedness;




  • dry mouth;




  • nausea, vomiting, stomach pain, loss of appetite;




  • feeling anxious or jittery;




  • drunk feeling; or




  • headache.

What other drugs will affect Medigesic (acetaminophen, butalbital, and caffeine)?


Tell your doctor if you regularly use other medicines that make you sleepy (such as cold or allergy medicine, narcotic pain medicine, sleeping pills, muscle relaxers, and medicine for seizures, depression, or anxiety). They can add to sleepiness caused by butalbital.

Tell your doctor about all other medicines you use, especially:



  • an antibiotic;




  • a blood thinner such as warfarin (Coumadin, Jantoven);




  • isoniazid (for treating tuberculosis);




  • zidovudine (Retrovir, AZT);




  • seizure medication such as phenytoin (Dilantin) or phenobarbital (Luminal, Solfoton);




  • gout medications such as probenecid (Benemid) or sulfinpyrazone;




  • steroids such as prednisone, fluticasone (Advair), mometasone (Asmanex, Nasonex), dexamethasone (Decadron, Hexadrol) and others; or




  • an antidepressant such as amitriptyline (Elavil, Vanatrip, Limbitrol), clomipramine (Anafranil), desipramine (Norpramin), imipramine (Janimine, Tofranil), and others.

4. Clinical Particulars



4.1 Therapeutic Indications



Erythromycin lactobionate is indicated in severe and immunocompromised cases of infections caused by sensitive organisms where high blood levels are required at the earliest opportunity or when the oral route is compromised.



1. Upper Respiratory Tract infections: tonsillitis, peritonsillar abscess, pharyngitis, laryngitis, sinusitis, secondary infections in influenza and common colds



2. Lower Respiratory Tract infections: tracheitis, acute and chronic bronchitis, pneumonia (lobar pneumonia, bronchopneumonia, primary atypical pneumonia), bronchiectasis, Legionnaire's disease



3. Ear infection: otitis media and otitis externa, mastoiditis



4. Oral infections: gingivitis, Vincent's angina



5. Eye infections: blepharitis



6. Skin and soft tissue infections: boils and carbuncles, paronychia, abscesses, pustular acne, impetigo, cellulitis, erysipelas



7. Gastrointestinal infections: cholecystitis, staphylococcal enterocolitis



8. Prophylaxis: peri-operative secondary infection prophylaxis, severe trauma and burns secondary infection prophylaxis, endocarditis prophylaxis (dental procedures)



9. Septicaemia



10. Endocarditis



11. Other infections: osteomyelitis, urethritis, gonorrhoea, syphilis, lymphogranuloma venereum, diphtheria, prostatitis, scarlet fever



4.2 Posology And Method Of Administration



Adults: severe and immunocompromised infections, 50mg/kg/day, preferably by continuous infusion (equivalent to 4g per day for adults).



Mild to moderate infections (oral route compromised) 25mg/kg/day.



Newborn infant (birth to 1 month): 10-15mg/kg 3 times daily.



Children: 12.5mg/kg 4 times daily. Doses can be doubled in severe infections.



Elderly: No special dosage recommendations.



Recommended Administration



Continuous intravenous infusion with an erythromycin concentration of 1mg/ml (0.1% solution) is recommended. The infusion should be completed within 8 hours of preparation to ensure potency.



If required, solution strengths up to 5mg/ml (0.5% solution) may be used, but should not be exceeded. Higher concentrations may result in pain along the vein.



Bolus injection is not recommended.



However, if it is decided to administer the daily dose as 4 doses once every 6 hours, then the erythromycin concentration should not exceed 5mg/ml and the time of each infusion should be between 20 and 60 minutes.



4.3 Contraindications



Known hypersensitivity to erythromycin. Erythromycin is contraindicated in patients taking astemizole, terfenadine, cisapride or pimozide.



Erythromycin is contraindicated with ergotamine and dihydroergotamine.



4.4 Special Warnings And Precautions For Use



Erythromycin is excreted principally by the liver, so caution should be exercised in administering the antibiotic to patients with impaired hepatic function or concomitantly receiving potentially hepatotoxic agents. Hepatic dysfunction including increased liver enzymes and/or cholestatic hepatitis, with or without jaundice, has been infrequently reported with erythromycin.



There have been reports suggesting erythromycin does not reach the foetus in adequate concentrations to prevent congenital syphilis. Infants born to women treated during pregnancy with oral erythromycin for early syphilis should be treated with an appropriate penicillin regimen.



There have been reports that erythromycin may aggravate the weakness of patients with myasthenia gravis.



Erythromycin interferes with the fluorometric determination of urinary catecholamines.



As with other broad spectrum antibiotics, pseudomembranous colitis has been reported rarely with erythromycin.



Rhabdomyolysis with or without renal impairment has been reported in seriously ill patients receiving erythromycin concomitantly with lovastatin.



Prolonged QTc interval and ventricular arrhythmias have rarely been reported in patients receiving erythromycin IV.



4.5 Interaction With Other Medicinal Products And Other Forms Of Interaction



Concomitant use of erythromycin with terfenadine or astemizole is likely to result in an enhanced risk of cardiotoxicity with these drugs. The concomitant use of erythromycin with either astemizole or terfenadine is therefore contraindicated.



The metabolism of terfenadine and astemizole is significantly altered when either are taken concomitantly with erythromycin. Rare cases of serious cardiovascular events have been observed, including torsades de pointes, other ventricular arrhythmias and cardiac arrest. Death has been reported with the terfenadine / erythromycin combination.



Elevated cisapride levels have been reported in patients receiving erythromycin and cisapride concomitantly. This may result in QT prolongation and cardiac arrhythmias including ventricular tachycardia, ventricular fibrillation and Torsades de pointes. Similar effects have been observed with concomitant administration of pimozide and clarithromycin, another macrolide antibiotic.



Mizolastine has a weak potential to prolong QT interval and has not been associated with arrhythmias, however, the metabolism of mizolastine is inhibited by erythromycin, therefore concomitant use should be avoided.



Concurrent use of erythromycin and ergotamine or dihydroergotamine has been associated in some patients with acute ergot toxicity characterised by the rapid development of severe peripheral vasospasm and dysaesthesia.



Increases in serum concentrations of the following drugs metabolised by the cytochrome P450 system may occur when administered concurrently with erythromycin: alfentanil, astemizole, bromocriptine, carbamazepine, cyclosporin, digoxin, dihydroergotamine, disopyramide, ergotamine, hexobarbitone, midazolam, phenytoin, quinidine, tacrolimus, terfenadine, theophylline, triazolam, valproate, acenocoumarol, rifabutin, and warfarin. Appropriate monitoring should be undertaken and dosage should be adjusted as necessary.



Erythromycin has been reported to decrease the clearance of zopiclone and thus may increase the pharmacodynamic effects of this drug.



When oral erythromycin is given concurrently with theophylline, there is also a significant decrease in erythromycin serum concentrations. The decrease could result in subtherapeutic concentrations of erythromycin.



4.6 Pregnancy And Lactation



There is no evidence of hazard from erythromycin in human pregnancy. It has been in widespread use for a number of years without apparent ill consequence. Animal studies have shown no hazard.



Erythromycin has been reported to cross the placental barrier in humans, but foetal plasma levels are generally low.



Erythromycin is excreted in breast milk, therefore, caution should be exercised when erythromycin is administered to a nursing mother.



4.7 Effects On Ability To Drive And Use Machines



None reported.



4.8 Undesirable Effects



Occasional side effects such as nausea, abdominal discomfort, vomiting and diarrhoea may be experienced. Reversible hearing loss associated with doses of erythromycin usually greater than 4g per day has been reported. Allergic reactions are rare and mild, although anaphylaxis has occurred. Skin reactions ranging from mild eruptions to erythema multiforme, Stevens-Johnson syndrome and toxic epidermal necrolysis have rarely been reported. There are no reports implicating erythromycin products with abnormal tooth development, and only rare reports of damage to the blood, kidneys or central nervous system.



Cardiac arrhythmias have been very rarely reported in patients receiving erythromycin therapy. There have been isolated reports of chest pain, dizziness and palpitations, however, a cause and effect relationship has not been established.



Symptoms of hepatitis, hepatic dysfunction and/or abnormal liver function test results may occur.



4.9 Overdose



Symptoms: hearing loss, severe nausea, vomiting and diarrhoea.



Treatment: gastric lavage, general supportive measures.



5. Pharmacological Properties



5.1 Pharmacodynamic Properties



Erythromycin exerts its antimicrobial action by binding to the 50S ribosomal sub-unit of susceptible microorganisms and suppresses protein synthesis. Erythromycin is usually active against most strains of the following organisms both in vitro and in clinical infections:



Gram positive bacteria - Listeria monocytogenes, Corynebacterium diphtheriae (as an adjunct to antitoxin), Staphylococci spp, Streptococci spp (including Enterococci).



Gram negative bacteria - Haemophilus influenzae, Neisseria meningitidis, Neisseria gonorrhoeae, Legionella pneumophila, Moraxella (Branhamella) catarrhalis, Bordetella pertussis, Campylobacter spp.



Mycoplasma - Mycoplasma pneumoniae, Ureaplasma urealyticum.



Other organisms -Treponema pallidum, Chlamydia spp, Clostridia spp, L-forms, the agents causing trachoma and lymphogranuloma venereum.



Note: The majority of strains of Haemophilus influenzae are susceptible to the concentrations reached after ordinary doses.



5.2 Pharmacokinetic Properties



Following intravenous infusion, erythromycin is widely distributed throughout body tissues, including lung tissues.

What is Triaminic Cough & Runny Nose Softchews (chlorpheniramine and dextromethorphan)?

Chlorpheniramine is an antihistamine. It blocks the effects of the naturally occurring chemical histamine in the body. Chlorpheniramine prevents sneezing; itchy, watery eyes and nose; and other symptoms of allergies and hay fever.


Dextromethorphan is a cough suppressant. It affects the signals in the brain that trigger cough reflex.


Dextromethorphan will not treat a cough that is caused by smoking, asthma, or emphysema.

The combination of chlorpheniramine and dextromethorphan is used to treat sneezing, itching, watery eyes, runny nose, and cough caused by allergies, the common cold, or the flu.


Chlorpheniramine and dextromethorphan may also be used for other purposes not listed in this medication guide.


What is the most important information I should know about Triaminic Cough & Runny Nose Softchews (chlorpheniramine and dextromethorphan)?


Always ask a doctor before giving a cough or cold medicine to a child. Death can occur from the misuse of cough and cold medicines in very young children. Do not use a cough or cold medicine if you have used an MAO inhibitor such as isocarboxazid (Marplan), phenelzine (Nardil), rasagiline (Azilect), selegiline (Eldepryl, Emsam), or tranylcypromine (Parnate) within the past 14 days. Serious, life-threatening side effects can occur if you take cough or cold medicine before the MAO inhibitor has cleared from your body. Avoid drinking alcohol while you are taking this medication. It can add to drowsiness caused by an antihistamine. Do not use any other over-the-counter cough, cold, allergy, or sleep medication without first asking your doctor or pharmacist. If you take certain products together you may accidentally take too much of one or more types of medicine. Read the label of any other medicine you are using to see if it contains an antihistamine or cough suppressant. Dextromethorphan will not treat a cough that is caused by smoking, asthma, or emphysema.

Artificially-sweetened liquid forms of this medicine may contain phenylalanine. This would be important to know if you have phenylketonuria (PKU). Check the ingredients and warnings on the medication label if you are concerned about phenylalanine.


How should I take Triaminic Cough & Runny Nose Softchews (chlorpheniramine and dextromethorphan)?


Use this medication exactly as directed on the label, or as it has been prescribed by your doctor. Do not use the medication in larger amounts, or use it for longer than recommended. Cold medicine is usually taken for only a short time until your symptoms clear up.


Always ask a doctor before giving cough or cold medicine to a child. Death can occur from the misuse of cough or cold medicine in very young children.

Measure the liquid form of this medicine with a special dose-measuring spoon or cup, not a regular table spoon. If you do not have a dose-measuring device, ask your pharmacist for one.


The chewable tablet should be chewed before you swallow it.


Talk with your doctor if your symptoms do not improve after 7 days of treatment, or if you have a fever with a headache, cough, or skin rash.

This medication can cause you to have unusual results with allergy skin tests. Tell any doctor who treats you that you are taking an antihistamine.


If you need to have any type of surgery, tell the surgeon ahead of time if you have taken a cold medicine within the past few days.


Store this medicine at room temperature, away from heat, light, and moisture.

What is aspirin and oxycodone?

Aspirin is in a group of drugs called salicylates (sa-LIS-il-ates). It works by reducing substances in the body that cause pain, fever, and inflammation.


Oxycodone is in a group of drugs called narcotic pain relievers.


The combination of aspirin and oxycodone is used to relieve moderate to severe pain.


Aspirin and oxycodone may also be used for purposes not listed in this medication guide.


What is the most important information I should know about aspirin and oxycodone?


Aspirin should not be given to a child or teenager who has a fever, especially if the child also has flu symptoms or chicken pox. Aspirin can cause a serious and sometimes fatal condition called Reye's syndrome in children.

Stop using this medication and call your doctor at once if you have any symptoms of bleeding in your stomach or intestines. Symptoms include black, bloody, or tarry stools, and coughing up blood or vomit that looks like coffee grounds.


Oxycodone may be habit-forming and should be used only by the person it was prescribed for. Never share aspirin and oxycodone with another person, especially someone with a history of drug abuse or addiction. Keep the medication in a place where others cannot get to it. Avoid drinking alcohol. It may increase your risk of stomach bleeding while taking aspirin.

Tell your doctor if the medicine seems to stop working as well in relieving your pain.


What should I discuss with my healthcare provider before taking aspirin and oxycodone?


Aspirin should not be given to a child or teenager who has a fever, especially if the child also has flu symptoms or chicken pox. Aspirin can cause a serious and sometimes fatal condition called Reye's syndrome in children. Do not use aspirin and oxycodone if you have a bleeding or blood clotting disorder such as hemophilia. Do not use this medication if you are allergic to aspirin or oxycodone or to a non-steroidal anti-inflammatory drug (NSAID) such as Advil, Aleve, Motrin, Naprosyn, Orudis, Cataflam, Celecoxib, Feldene, Indocin, Lodine, Mobic, Relafen, Toradol, Voltaren, and others.

To make sure you can safely take this medicine, tell your doctor if you have any of these other conditions:



  • asthma or other breathing disorders;



  • liver or kidney disease;


  • a history of head injury or brain tumor;




  • epilepsy or other seizure disorder;




  • stomach or intestinal disorder, history of stomach ulcer or bleeding;




  • a pancreas disorder;




  • curvature of the spine; or




  • mental illness or a history of drug or alcohol addiction.




Oxycodone may be habit-forming and should be used only by the person it was prescribed for. Never share aspirin and oxycodone with another person, especially someone with a history of drug abuse or addiction. FDA pregnancy category D. This medication can cause harm to an unborn baby, and breathing problems or addiction/withdrawal symptoms in a newborn. Taking aspirin during late pregnancy may cause bleeding in the mother or the baby during delivery. Do not take aspirin and oxycodone during pregnancy unless your doctor has told you to. Use an effective form of birth control, and tell your doctor if you become pregnant during treatment. Aspirin and oxycodone can pass into breast milk and may harm a nursing baby. Do not use this medication without telling your doctor if you are breast-feeding a baby.

Older adults may be more sensitive to the effects of this medicine.


How should I take aspirin and oxycodone?


Take exactly as prescribed by your doctor. Do not take in larger or smaller amounts or for longer than recommended. Follow the directions on your prescription label. Tell your doctor if the medicine seems to stop working as well in relieving your pain.


Drink 6 to 8 full glasses of water daily to help prevent constipation while you are taking aspirin and oxycodone. Ask your doctor about ways to increase the fiber in your diet. Do not use a stool softener (laxative) without first asking your doctor. You may have withdrawal symptoms when you stop using this medication after using it over a long period of time. Do not stop using the medication suddenly without first talking to your doctor. You may need to use less and less before you stop the medication completely. If you need surgery, tell the surgeon ahead of time that you are using aspirin and oxycodone. Store at room temperature away from moisture, heat, and light.

Keep track of the amount of medicine used from each new bottle. Oxycodone is a drug of abuse and you should be aware if anyone is using your medicine improperly or without a prescription.


Always check your bottle to make sure you have received the correct pills (same brand and type) of medicine prescribed by your doctor. Ask the pharmacist if you have any questions about the medicine you receive at the pharmacy.


After you have stopped using this medication, flush any unused pills down the toilet.

What happens if I miss a dose?


Since aspirin and oxycodone is taken as needed, you may not be on a dosing schedule. If you are taking the medication regularly, take the missed dose as soon as you remember. Skip the missed dose if it is almost time for your next scheduled dose. Do not take extra medicine to make up the missed dose.


What happens if I overdose?


Seek emergency medical attention or call the Poison Help line at 1-800-222-1222. An overdose of aspirin and oxycodone can be fatal.

Overdose symptoms may include extreme drowsiness, pinpoint pupils, nausea, vomiting, diarrhea, confusion, ringing in your ears, cold or clammy skin, muscle weakness, fainting, weak pulse, slow heart rate, coma, blue lips, shallow breathing, or no breathing.


What should I avoid while taking aspirin and oxycodone?


Aspirin and oxycodone may impair your thinking or reactions. Be careful if you drive or do anything that requires you to be alert. Ask a doctor or pharmacist before using any other over-the-counter cold, allergy, or pain medicine. Aspirin is contained in many combination medicines. Taking certain products together can cause you to get too much aspirin. Check the label to see if a medicine contains aspirin. Avoid drinking alcohol. It may increase your risk of stomach bleeding while taking aspirin.

Aspirin and oxycodone side effects


Get emergency medical help if you have any of these signs of an allergic reaction: hives; difficulty breathing; swelling of your face, lips, tongue, or throat. Call your doctor at once if you have a serious side effect such as:

  • black, bloody, or tarry stools, coughing up blood or vomit that looks like coffee ground;




  • shallow breathing, slow heartbeat;




  • fast heart rate;




  • feeling light-headed, fainting;




  • confusion, hallucinations;




  • easy bruising or bleeding; or




  • problems with urination.



Less serious side effects may include:



  • headache, dizziness, drowsiness;




  • heartburn, nausea, vomiting, upset stomach, bloating, gas, constipation, diarrhea;




  • feeling dizzy or drowsy;




  • headache;




  • sweating;




  • ringing in your ears; or




  • dry mouth.

Aspirin and oxycodone Dosing Information


Usual Adult Dose for Pain:

For the relief of moderate to moderately severe pain:
1 tablet (aspirin 325 mg/oxycodone 4.5-0.38 mg or aspirin 325 mg/oxycodone 4.8355 mg) or
1 to 2 tablets (aspirin 325 mg/oxycodone 2.25-0.198 mg) orally every 6 hours.

Usual Pediatric Dose for Pain:

5 years or younger: Use is not indicated.

6 to 12 years: One-fourth of an aspirin-oxycodone 325 mg-2.25 mg-0.19 mg tablet every six hours.

12 years or older: One-half of an aspirin-oxycodone 325 mg-2.25 mg-0.19 mg tablet every six hours.


What other drugs will affect aspirin and oxycodone?


Before using this medication, tell your doctor if you regularly use other medicines that make you sleepy or could slow your breathing (such as cold or allergy medicine, sedatives, narcotic pain medicine, sleeping pills, muscle relaxers, and medicine for seizures, depression, or anxiety). They can add to sleepiness caused by aspirin and oxycodone.

Tell your doctor about all other medicines you use, especially:



  • acetazolamide (Diamox);




  • a diuretic (water pill);




  • methotrexate (Rheumatrex, Trexall);




  • steroids (prednisone and others);




  • a blood thinner such as warfarin (Coumadin, Jantoven);




  • a bronchodilator (such as Atrovent, Spiriva);




  • atropine (Donnatal), dimenhydrinate (Dramamine), or scopolamine (Transderm-Scop);




  • insulin or diabetes medications that you take by mouth;




  • an ACE inhibitor such as benazepril (Lotensin), enalapril (Vasotec), lisinopril (Prinivil, Zestril), quinapril (Accupril), ramipril (Altace), and others;




  • a beta-blocker such as atenolol (Tenormin, Tenoretic), carvedilol (Coreg), labetalol (Normodyne, Trandate), metoprolol (Lopressor, Toprol), nadolol (Corgard), propranolol (Inderal, InnoPran), sotalol (Betapace), and others;




  • bowel or bladder medications such as dicyclomine (Bentyl), hyoscyamine (Anaspaz, Cystospaz, Levsin), tolterodine (Detrol) and others; or




  • an NSAID such as ibuprofen (Motrin, Advil), diclofenac (Cataflam, Celecoxib, Voltaren), etodolac (Lodine), indomethacin (Indocin), meloxicam (Mobic), nabumetone (Relafen), naproxen (Aleve, Naprosyn), piroxicam (Feldene), and others.

Qvar 100 Easi-Breathe





1. Name Of The Medicinal Product



Qvar Easi-Breathe 100 micrograms per actuation pressurised inhalation solution


2. Qualitative And Quantitative Composition



Beclometasone Dipropionate 100 micrograms per metered (ex-valve) dose.



(The dose delivered from the mouthpiece is an average 75 micrograms).

3. Pharmaceutical Form



Pressurised inhalation, solution.



A colourless solution in a pressurised aluminium canister fitted with a metering valve and an actuator.



Qvar Easi-Breathe contains a propellant, which does not contain any chlorofluorocarbons (CFCs).



4. Clinical Particulars



4.1 Therapeutic Indications



Prophylactic management of mild, moderate or severe asthma.



4.2 Posology And Method Of Administration



Qvar Easi-Breathe is for inhalation use only.



Patients should be instructed in the proper use of their inhaler, including rinsing out their mouth with water after use. Patients should be advised that Qvar may have a different taste and feel than a CFC inhaler.



NOTE: The recommended total daily dose of Qvar Easi-Breathe is lower than that for current beclometasone dipropionate CFC containing products and should be adjusted to the needs of the individual patient.



ADULT STARTING AND MAINTENANCE DOSE:



It is important to gain control of asthma symptoms and optimise pulmonary function as soon as possible. When patients' symptoms remain under satisfactory control, the dose should be titrated to the lowest dose at which effective control of asthma is maintained.



To be effective inhaled Qvar Easi-Breathe must be used on a regular basis even when patients are asymptomatic.



THERAPY IN NEW PATIENTS SHOULD BE INITIATED AT THE FOLLOWING DOSAGES










Mild asthma:




100 to 200 micrograms per day in two divided doses.




Moderate asthma:




200 to 400 micrograms per day in two divided doses.




Severe asthma:




400 to 800 micrograms per day in two divided doses.



TRANSFERRING PATIENTS TO QVAR EASI-BREATHE FROM A CFC-CONTAINING INHALER



The general approach to switching patients to Qvar Easi-Breathe involves two steps as detailed below. Specific guidance on switching well-controlled and poorly-controlled (symptomatic) patients is given below the table.



Step 1: Consider the dose of CFC containing beclometasone dipropionate product appropriate to the patient's current condition.



Step 2: Convert the CFC containing beclometasone dipropionate dose to the Qvar Easi-Breathe dose according to the table below.

1. Dosing in well-controlled patients with asthma



Patients with well-controlled asthma using beclometasone dipropionate CFC containing product should be switched to Qvar Easi-Breathe at a dose in accordance with the table above.



For example:



Patients on 2 puffs twice daily of CFC beclometasone dipropionate 200 micrograms would change to 2 puffs twice daily of Qvar Easi-Breathe 100 micrograms.



2. Dosing in poorly-controlled (symptomatic) patients with asthma



Patients with poorly-controlled asthma may be switched from CFC containing beclometasone dipropionate products to Qvar Easi-Breathe at the same microgram for microgram dose up to 800 micrograms daily. Comparative clinical studies have demonstrated that asthma patients achieve equivalent pulmonary function and control of symptoms with Qvar aerosol (an equivalent inhaler) at lower total daily doses than with CFC containing beclometasone dipropionate products.



Alternatively the patient's current CFC containing beclometasone dipropionate dose can be doubled and this dose can be converted to the Qvar Easi-Breathe dose according to the table above.



Patients on budesonide inhalers may be transferred to Qvar Easi-Breathe as described for CFC containing beclometasone dipropionate products.



Patients on fluticasone inhalers may be transferred to the same total daily dose of Qvar Easi-Breathe up to 800 micrograms daily.



Once transferred to Qvar Easi-Breathe the dose should be adjusted to meet the needs of the individual patient.



The maximum recommended dose is 800 micrograms per day in divided doses.



The same total daily dose in micrograms from either Qvar Easi-Breathe 50 (a lower strength) or Qvar Easi-Breathe 100 Inhaler provides the same clinical effect.



Patients should be instructed in the proper use of their inhaler, including rinsing out their mouth with water after use. Patients should be advised that Qvar Easi-Breathe may have a different taste and feel than a CFC inhaler.



CHILDREN



There are no data to date on Qvar Easi-Breathe in children under 12 years of age, hence no definitive dosage recommendation can be made.



SPECIAL PATIENT GROUPS



No special dosage recommendations are made for elderly or patients with hepatic or renal impairment.



INSTRUCTIONS FOR USE



The aerosol spray is inhaled through the mouth into the lungs. The inhaler should be primed by firing two shots into the air before first use or if the inhaler has not been used for a period of two weeks or longer.



After removal of the cap the inhaler mouthpiece should be placed in the mouth with the lips closed around it. The patient should breathe in slowly and deeply through the mouthpiece. They should be advised not to stop breathing when the inhaler delivers the dose into their mouth but carry on until they have taken a deep breath to ensure optimal delivery of the product.



For normal hygiene, the mouthpiece of the inhaler should be cleaned weekly with a clean dry tissue or cloth. The inhaler should not be washed or immersed in water at any time.

Qvar Easi-Breathe delivers a consistent dose



- whether or not the canister is shaken by the patient



- without the need for the patient to wait between individual actuations



- regardless of storage orientation or periods without use of up to 14 days



- at temperatures as low as -10°C.



4.3 Contraindications



Hypersensitivity to beclometasone dipropionate or to any of the excipients.



4.4 Special Warnings And Precautions For Use



To be effective, Qvar Easi-Breathe must be used by patients on a regular basis, even when patients do not have asthma symptoms. When symptoms are controlled, maintenance Qvar Easi-Breathe therapy should be reduced in a stepwise manner to the minimum effective dose. Inhaled steroid treatment should not be stopped abruptly.



Patients with asthma are at risk of acute attacks and should have regular assessments of their asthma control including pulmonary function tests.



Qvar Easi-Breathe is not indicated for the immediate relief of asthma attacks. Patients therefore need to have relief medication (inhaled short-acting bronchodilator) available for such circumstances.



Qvar Easi-Breathe is not indicated in the management of status asthmaticus.



Severe asthma exacerbations should be managed in the usual way. Subsequently, it may be necessary to increase the dose of Qvar Easi-Breathe up to the maximum daily dose. Systemic steroid treatment may be needed and/or an antibiotic, if there is an infection.



Patients should be advised to seek medical attention for review of maintenance Qvar Easi-Breathe therapy if peak flow falls, symptoms worsen or if the short-acting bronchodilator becomes less effective and increased inhalations are required. This may indicate worsening asthma.



Patients who have received systemic steroids for long periods of time or at high doses, or both, need special care and subsequent management when being transferred to inhaled steroid therapy. Patients should have stable asthma before being given inhaled steroids in addition to the usual maintenance dose of systemic steroid. Withdrawal of systemic steroids should be gradual, starting about seven days after the introduction of Qvar Easi-Breathe therapy. For daily oral doses of prednisolone of 10mg or less, dose reduction in 1mg steps, at intervals of not less than one week is recommended. For patients on daily maintenance doses of oral prednisolone greater than 10mg, larger weekly reductions in the dose might be acceptable. The dose reduction scheme should be chosen to correlate with the magnitude of the maintenance systemic steroid dose.



Most patients can be successfully transferred to inhaled steroids with maintenance of good respiratory function, but special care is necessary for the first few months after the transfer, until the hypothalamic-pituitary-adrenal (HPA) system has sufficiently recovered to enable the patient to cope with stressful emergencies such as trauma, surgery or serious infections. Patients should, therefore, carry a steroid warning card to indicate the possible need to re-instate systemic steroid therapy rapidly during periods of stress or where airways obstruction or mucus significantly compromises the inhaled route of administration. In addition, it may be advisable to provide such patients with a supply of corticosteroid tablets to use in these circumstances. The dose of inhaled steroids should be increased at this time and then gradually reduced to the maintenance level after the systemic steroid has been discontinued. As recovery from impaired adrenocortical function, caused by prolonged systemic steroid therapy is slow, adrenocortical function should be monitored regularly.



Patients should be advised that they may feel unwell in a non-specific way during systemic steroid withdrawal despite maintenance of, or even improved respiratory function. Patients should be advised to persevere with their inhaled product and to continue withdrawal of systemic steroids, even if feeling unwell, unless there is evidence of HPA axis suppression.



Discontinuation of systemic steroids may also cause exacerbation of allergic diseases such as atopic eczema and rhinitis. These should be treated as required with topical therapy, including corticosteroids and/or antihistamines.



Beclometasone dipropionate, like other inhaled steroids, is absorbed into the systemic circulation from the lungs. Beclometasone dipropionate and its metabolites may exert detectable suppression of adrenal function. Within the dose range 100-800 micrograms daily, clinical studies with Qvar aerosol (an equivalent inhaler) have demonstrated mean values for adrenal function and responsiveness within the normal range. However, systemic effects of inhaled corticosteroids may occur, particularly at high doses prescribed for prolonged periods. These effects are much less likely to occur than with oral corticosteroids. Possible systemic effects include Cushing's syndrome, Cushingoid features, adrenal suppression, growth retardation in children and adolescents, decrease in bone mineral density, cataract, glaucoma, and more rarely, a range of psychological or behavioural effects including psychomotor hyperactivity, sleep disorders, anxiety, depression or aggression (particularly in children). It is important, therefore, that the dose of inhaled corticosteroid is titrated to the lowest dose at which effective control of asthma is maintained.



It is recommended that the height of children receiving prolonged treatment with inhaled corticosteroids is regularly monitored. If growth is slowed, therapy should be reviewed with the aim of reducing the dose of inhaled corticosteroid, if possible, to the lowest dose at which effective control of asthma is maintained. In addition, consideration should be given to referring the patient to a paediatric respiratory specialist.



Prolonged treatment with high doses of inhaled corticosteroids, particularly higher than the recommended doses, may result in clinically significant adrenal suppression. Additional systemic corticosteroid cover should be considered during periods of stress or elective surgery.



Like other corticosteroids, caution is necessary in patients with active or latent pulmonary tuberculosis.



4.5 Interaction With Other Medicinal Products And Other Forms Of Interaction



No interaction studies have been performed.



4.6 Pregnancy And Lactation



The potential risk of this product for humans is unknown.



Qvar Easi-Breathe



There is no experience of this product in pregnancy and lactation in humans, therefore the product should only be used if the expected benefits to the mother are thought to outweigh any potential risk to the foetus or neonate.



Beclometasone dipropionate



There is inadequate evidence of safety in human pregnancy.



The use of beclometasone dipropionate in pregnancy requires that the possible benefits of the drug be weighed against the possible hazards. The drug has been in widespread use for many years without apparent ill consequence.



It is probable that beclometasone dipropionate is excreted in milk. However, given the relatively low doses used by the inhalation route, the levels are likely to be low. In mothers breast-feeding their baby the therapeutic benefits of the drug should be weighed against the potential hazards to mother and baby.



4.7 Effects On Ability To Drive And Use Machines



Not relevant.



4.8 Undesirable Effects



A serious hypersensitivity reaction including oedema of the eye, face, lips and throat (angioedema) has been reported rarely.



As with other inhaled therapy, paradoxical bronchospasm may occur after dosing. Immediate treatment with a short-acting bronchodilator should be initiated, Qvar should be discontinued immediately and an alternate prophylactic treatment introduced.



Systemic effects of inhaled corticosteroids may occur, particularly with high doses prescribed for prolonged periods. These include adrenal suppression, growth retardation in children, decrease in bone mineral density and the occurrence of cataract and glaucoma.



Commonly, when taking Qvar, hoarseness and candidiasis of the throat and mouth may occur. To reduce the risk of hoarseness and candida infection, patients are advised to rinse their mouth after using their inhaler.

4.9 Overdose



Acute overdosage is unlikely to cause problems. The only harmful effect that follows inhalation of large amounts of the drug over a short time period is suppression of HPA function. Specific emergency action need not be taken. Treatment with Qvar Easi-Breathe should be continued at the recommended dose to control the asthma; HPA function recovers in a day or two.



If excessive doses of beclometasone dipropionate were taken over a prolonged period a degree of atrophy of the adrenal cortex could occur in addition to HPA suppression. In this event the patient should be treated as steroid dependent and transferred to a suitable maintenance dose of a systemic steroid such as prednisolone. Once the condition is stabilised, the patient should be returned to Qvar Easi-Breathe by the method described above in Section 4.4.



5. Pharmacological Properties



5.1 Pharmacodynamic Properties



Pharmacotherapeutic group: Glucocorticoids, ATC code: R03BA01



Qvar Easi-Breathe contains beclometasone dipropionate in solution in propellant HFA-134a resulting in an extra fine aerosol. The aerosol droplets are on average much smaller than the beclometasone dipropionate particles delivered by CFC-suspension formulations or dry powder formulations of beclometasone dipropionate. The extra fine particle fraction will be 60% ± 20% of the drug particles



Radiolabelled deposition studies in adults with mild asthma have demonstrated that the majority of drug (> 55% ex-actuator) is deposited in the lung and a small amount (< 35% ex-actuator) is deposited in the oropharynx. These studies were performed with Qvar Aerosol. Qvar Aerosol is a 'press and breathe' inhaler, whereas Qvar Easi-Breathe is a breath-activated inhaler.



Inhaled beclometasone dipropionate is now well established in the management of asthma. It is a synthetic glucocorticoid and exerts a topical, anti-inflammatory effect on the lungs, with fewer systemic effects than oral corticosteroids.



Comparative clinical studies of Qvar aerosol have demonstrated that asthma patients achieve equivalent pulmonary function and control of symptoms with Qvar aerosol at lower total daily doses than CFC containing beclometasone dipropionate aerosol inhalers.



Pharmacodynamic studies in patients with mild asthma given Qvar aerosol for 14 days, have shown that there is a linear correlation among urinary free cortisol suppression, dose administered, and serum total-beclometasone levels obtained. At a daily dose of 800 micrograms Qvar aerosol, suppression of urinary free cortisol was comparable with that observed with the same daily dose of CFC containing beclometasone dipropionate, indicating a wider safety margin, as Qvar Easi-Breathe is administered at lower doses than the CFC product.



5.2 Pharmacokinetic Properties



The pharmacokinetic profile of Qvar aerosol (an equivalent inhaler) shows that the peak serum concentration for total- beclometasone (BOH) (total of any beclometasone OH and beclometasone dipropionate or monopropionate hydrolysed to beclometasone OH) or after single and multiple doses is achieved after 30 minutes.



The value at the peak is approximately 2 nanograms/ml after a total daily dose of 800 micrograms and the serum levels after 100, 200 and 400 micrograms are proportional. The principal route of elimination of beclometasone dipropionate and its several metabolites is in the faeces. Between 10% and 15% of an orally administered dose is excreted in the urine, as both conjugated and free metabolites of the drug.



In both single dose and multiple dose pharmacokinetic studies of Qvar aerosol, a dose of 200 micrograms of Qvar aerosol achieved comparable total-BOH levels, as a dose of 400 micrograms of CFC containing beclometasone dipropionate Aerosol. This provided the scientific rationale for investigating lower total daily doses of Qvar aerosol to achieve the same clinical effect.



Pharmacokinetic studies with Qvar Easi-Breathe have not been carried out in any special populations.



5.3 Preclinical Safety Data



In animal studies, propellant HFA-134a has been shown to have no significant pharmacological effects other than at very high exposure concentrations, then narcosis and a relatively weak cardiac sensitising effect were found. The potency of the cardiac sensitisation was less than that of CFC-11 (trichlorofluoromethane).



In studies to detect toxicity, repeated high dose levels of propellant HFA-134a indicated that safety margins based on systemic exposure would be of the order 2200, 1314 and 381 for mouse, rat and dog with respect to humans.



There are no reasons to consider propellant HFA-134a as a potential mutagen, clastogen or carcinogen judged from in vitro and in vivo studies including long-term administration by inhalation in rodents.



Studies of propellant HFA-134a administered to pregnant and lactating rats and rabbits have not revealed any special hazard.



In animals, systemic administration of relatively high doses can cause abnormalities of foetal development including growth retardation and cleft palate. There may therefore be a very small risk of such effects in the human foetus. However, inhalation of beclometasone dipropionate into the lungs avoids the high level of exposure that occurs with administration by systemic routes.



Safety studies with Qvar aerosol (an equivalent inhaler) in rat and dog showed few, if any, adverse effects other than those normally associated with general steroid exposure including lymphoid tissue alterations such as reduction in thymus, adrenal and spleen weights. An inhalation reproductive study Qvar aerosol (an equivalent inhaler) in rats did not exhibit any teratogenic effects.
